An opportunity has arisen for a highly motivated individual to support the Intermediate Tier Service (ITS) at Rochdale Infirmary. Working with either the Urgent Community Care Team/ Tudor Court/ Wolstenholme

You will work within the newly integrated Intermediate Tier Services, within Pennine Acute Hospitals NHS Trust supporting our partnership with colleagues across health and social care, to provide assessment and treatment for patients within the ITS. This post offers you to work as a trusted assessor to support the patient from hospital/home or ITS services.

This new integrated service model offers the opportunity to work flexibly within a multi-disciplinary team across the Pennine Acute footprint and local Heywood, Middleton and Rochdale community, truly putting patients and service users first and offering care in the place best suited to their needs.

You must have appropriate UK professional registration.
